Consent for Grooming

Your consent is required to perform any and all grooming services in the HydroDog Teska LLC Mobile Grooming vehicle.

The deadline to sign the Consent for Grooming Services form and to submit proof of rabies documentation is the day prior to the appointment time. Failure to do so will result in a canceled appointment.

Health Conditions, no matter how minor, must be disclosed prior to the appointment. Some examples of health conditions include but are not limited to: Allergies, Heart Conditions, Arthritis, Seizures, Cancer, Auto Immune Diseases, etc.

De-matting or complete coat removal will dramatically alter your pet's appearance. This procedure may expose pre-existing health/skin problems for which HydroDog Teska LLC cannot be held liable. You authorize HydroDog Teska LLC to shave down your dog in the event of severe matting that cannot be brushed out or in the event that you request what is commonly referred to as a 'summer cut'. Both of these situations require the use of a short blade in order to remove the coat.

HydroDog Teska LLC reserves the right to discontinue the groom at any time if it becomes clear that it is not safe to continue. HydroDog Teska LLC is not liable for complications that may arise during the grooming process from receiving medication. HydroDog Teska LLC does not administer any medication or OTC calming aids.

Aggressive dogs may need to be muzzled and restrained as necessary for both dog and groomer safety.

Customers are responsible for flea and tick treatments, please notify the groomer before hand if fleas are present or suspected of being present so precautions can be made. HydroDog Teska LLC can do flea shampoo treatments at an additional cost, but it will be less effective if the whole residence is not treated. Flea treatments are only used in the event fleas are discovered during the grooming process. No matter the product, (be it "natural" or not) there are risks to both. HydroDog Teska LLC is not liable for any potential side effects from the use of Flea & Tick shampoo.

Rabies vaccine must be up to date, as required by Massachusetts law. HydroDog Teska LLC requires a visual proof of the rabies document which shows the exact expiration date, (month, day, year). While not required, HydroDog Teska LLC highly recommends dogs be fully vaccinated above and beyond the Commonwealth’s minimum rabies requirement as suggested by your veterinarian. Grooming services cannot be performed on the same day that vaccines are administered.

Payment is to be made upon completion of the service. HydroDog Teska LLC accepts all major credit cards, debit cards, cash and personal checks. Additional fees for Coat Quality, Temperament, and/or Flea Treatment will be applied as necessary to the final bill.

Cancellation and rescheduling of an appointment any later than 48 hours before the scheduled appointment will result in a $35 fee. Cancellations and no-shows on the day of the appointment will result in being billed the full amount of the appointment. It is considered a "no-show" when the client is not available at the scheduled appointment time and does not contact HydroDog Teska LLC that they are unable to keep the appointment. Please make every effort to call and cancel or reschedule when possible to avoid such situations.

Cancellations and No-Shows will be invoiced via electronic invoice and must be paid before rescheduling will be done. If not able to accept an electronic invoice via email, other arrangements will be made. Paper forms will be used as a last resort due to the delays in mailing.

Inclement weather or truck break downs happen. You will be offered the next soonest available appointment. Driveways & walkways must be cleared free of snow and ice prior to arrival. If upon arrival, these areas appear to be unsafe to park or walk, your pup’s appointment will be rescheduled to a different day and you will be charged $50 trip charge. If you are unable to sufficiently have all areas cleared in time and notify me prior to my arrival, I will simply reschedule your appointment to a later date and no fees or charges will be made.

Illness of client or groomer: I will notify all customers scheduled as soon as it is possible if I am ill. I ask that the customer does the same. If you, a family member, or your pup is sick, please contact HydroDog Teska LLC as soon as possible.

Photos and video may be taken and used on the HydroDog Teska LLC website and social media for promotional purposes.

HydroDog Teska LLC reserves the right to refuse service to any dog or client for any reason

You are only required to sign this form once per customer for the life of the dog(s) unless this form is updated, in which case a new one will be sent prior to the next appointment.
